The Bias Report Form is an online form where students can make the Bias Resources and Response Team aware of a bias incident. This form also has further information on who to contact regarding a bias incident. You may also call the bias hotline number provided to leave a message. The form can be found at the bottom of the Bias Response Team webpage.

A bias incident is an incident motivated, in whole or in part, by the offender's bias against the actual or perceived age, creed, color, disability, gender, gender identity, gender expression, national origin, race, religion, sex, sexual orientation, or any other protected status* of the targeted person or group, and may or may not rise to the level of a criminal offense.
